Requirements / Qualifications Required:
General Requirements:
• Graduated from Faculty / Department with accreditation score minimum B (except for the province on the island of Kalimantan, Maluku, Nusa Tenggara, and Papua; minimum C)
• Maximum age 25 years old Associate's degree (D3), 27 years old for Bachelor degree (S1), 32 years old for Doctor (S2); by December 31, 2014
• Marital status: not married, except for doctor applicants
• Minimum GPA 2.75 for State University graduate; 3.00 for Private University graduate; and 2.50 for doctors, both graduated from State or Private University
• Minimum height: 155 cms for female and 160 cms for male
• Computer literated
• Willing to be placed throughout Indonesia
• Willing to undergo 2 years employment bond, Education certificate will be held by BPJS Kesehatan
• Willing to not get married for 1 year, except for applicants when applying for doctor position who has been married

Specific Requirements:

• Educational background required: General Practitioners Profession
• Surat Keterangan Selesai Masa Bakti for post PTT doctor
• Graduated from Medical School / Faculty of State University
• Graduated from Medical School / Faculty of Private University with minimum accreditation score B
• Architect and Civil Engineering Applicants, Application Code (SDS) - Please use Red Folder
• Educational background required: possess Associate's / Bachelor's degree majoring in Architecture or Civil Engineering
• Able to draw 2D and 3D drawing using computer software

• Applicant are invited to register themselves directly (datang langsung) to local BPJS Kesehatan office and bring the application files in the folder with color as listed above and write the application code in the top right corner of the folder, which consists of:
• Curriculum Vitae / Résumé Data (include marital status and height)
• Copy of National Identity Card (KTP)
• Copy of education diploma which has been legalized by the University / College
• Copy of transcript which have been legalized
• Photograph size 4x6 with blue background 2 pieces
• Criminal Note Certificate (SKCK) from the local police department
• Employment Certificate of the previous work, if ever worked
• Supplement: other files accordance with specific requirements
